CHOICE SCHOLARSHIP/SGO'S/PARISH TUITION ASSISTANCE

- Nativity Catholic School Tuition Assistance Application Form

Nativity Catholic School has a long history of making Catholic Education affordable for a widely diverse
community. Nativity Catholic School and Parish are pleased to continue this tradition for the 2022-23 school
year. Since tuition alone does not cover the cost of a Nativity education, all families receive some support
from the yearly financial investment from Nativity parish. Each school year, Nativity parishioners support the
school with nearly $200,000.

Qualifying for Tuition Assistance
• A student must be currently registered for the next academic year.
• A student’s account must be in good standing.

Available Tuition Assistance Opportunities

Choice Scholarship (Voucher) Program (Grades 1-8)
Families who qualify for the Choice Scholarship Program (School Voucher) will be required to sign additional
forms for the Indiana Department of Education after the application is accepted. A copy of your 2021 Federal
Income Tax Return (pages with dependents listed and the adjusted gross income) must be submitted with
the application. The income guidelines to be eligible to qualify for a new voucher are:

In addition to the financial guidelines, a student must qualify by one of the following eligibility tracks:
Special Education Track, "F" Public School Track, Previous Choice Scholarship Student Track, Previous SGO
Track, Two Semesters in a Public School Track or Sibling Track.

Continuing Choice Voucher
An application must be submitted each year to continue the Choice Scholarship Voucher. A copy of your 2021 Federal Income Tax Return (pages with dependents listed and the adjusted gross income) must be submitted with the application. The income guidelines for the continued voucher are:

Scholarship Granting Organization (SGO Grant)
For families that are seeking tuition assistance but do not qualify for the School Choice (Voucher) Program, tuition assistance is available through SGO Grants. Tuition assistance funds are limited, so it is critical that you complete and submit the application with your 2021 Federal Income Tax Return as early as possible. Private donations make SGO grants possible through the Institute for Quality Education. The income guidelines to be eligible to qualify for an SGO grant are:

Tuition Assistance from Nativity Parish
Nativity Parish offers tuition assistance families that do not qualify for one of the above options. Funds for additional tuition assistance are raised through interest earned from the Nativity Educational Endowment Fund and donations from private donors. Funds are limited, so it is critical that you complete and submit the application with your 2021 Federal Income Tax Return as early as possible. Award Notification Families will be notified of tuition grants by email. All information received is held in strict confidence. Families receiving assistance are asked to observe the same strict confidentiality.
